Case Manager - follow-up and outcomes (remote)

We are growing our team at Ironwood Pine Community Partners Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Flexibility and kindness are expected. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for food distribution. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Milwaukee. • We reply to every applicant.
