Case Manager - follow-up and outcomes (remote)

We are growing our team at Foxglove Lantern Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a large organisation of about 208 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for licensing records. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing records. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in Tampa. • Applications are reviewed on a rolling basis.
