Case Manager - follow-up and outcomes (remote)

Clearpath Basin Civic Services Group supports public agencies in Las Vegas with budgeting, records and programmes. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for procurement. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with procurement. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with quarterly visits in Las Vegas. • We will tell you what to expect at each step of the hiring process.
