Case Manager - follow-up and outcomes (remote)

We are growing our team at Fernwood River Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Collaboration between teachers and staff is strong. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for enrolment. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with quarterly visits in Denver. • We reply to every applicant.
