Case Manager - follow-up and outcomes (remote)

Foxglove Wilder Early Learning runs childcare centres in Charlotte caring for infants, toddlers and preschoolers. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 156 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Fully remote, with a monthly team day in Charlotte. • We will tell you what to expect at each step of the hiring process.
