Case Manager - follow-up and outcomes (remote)

Foxglove Willow Housing provides affordable homes and support services around Milwaukee, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for repair requests. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Part-time, about 24 hours a week, with flexible days. • Remote-friendly, with a monthly team day in Milwaukee. • Applications are reviewed on a rolling basis.
