Case Manager - follow-up and outcomes (remote)

We are growing our team at Harborlight Ashton Health Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for patient education. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Raleigh. • We will tell you what to expect at each step of the hiring process.
