Case Manager - follow-up and outcomes (remote)

Nova Sable Early Learning operates neighbourhood childcare around Rochester, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for licensing records. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing records. Working arrangement • Contract for nine months, with a review at the end and a chance of extension. • Fully remote, with a monthly team day in Rochester. • We will tell you what to expect at each step of the hiring process.
