Case Manager - follow-up and outcomes (remote)

We are growing our team at Bright Rook Community Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. We are small enough to know everyone's name. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for client intake. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client intake. Working arrangement • Part-time, about 16 hours a week, afternoons. • Remote-friendly, with a monthly team day in Salt Lake City. • We will tell you what to expect at each step of the hiring process.
