Case Manager - follow-up and outcomes (remote)

We are growing our team at Summit Amber Community Partners Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 24 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for food distribution. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Contract for twelve months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in Phoenix. • We will tell you what to expect at each step of the hiring process.
