Case Manager - follow-up and outcomes (remote)

We are growing our team at Clearpath Wren Health and adding a Case Manager whose main responsibility will be follow-up and outcomes. Clinicians and administrators work side by side. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Contract for eight months, with clear deliverables from the first week. • Fully remote, with quarterly visits in St. Louis. • We will tell you what to expect at each step of the hiring process.
