Case Manager - follow-up and outcomes (remote)

Lumen Wilder Learning Group operates schools and learning programmes around Colorado Springs. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for student accounts. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with student accounts. Working arrangement • Full-time, on a regular day shift. • Fully remote, with a monthly team day in Colorado Springs. • We will tell you what to expect at each step of the hiring process.
