Case Manager - follow-up and outcomes (remote)

Foxglove Harbor Health provides primary and specialty care in Spokane, and we need a Case Manager to lead our follow-up and outcomes. Clinicians and administrators work side by side. What you will do • Help with applications and appointments for insurance authorisations. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with insurance authorisations. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Remote-friendly, with an occasional day on site in Spokane. • Applications are reviewed on a rolling basis.
