Case Manager - follow-up and outcomes (remote)

Fernwood Loom Property Management is a Savannah property manager with leasing, maintenance and resident services. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 191 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for unit turnovers. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with an occasional day on site in Savannah. • We will tell you what to expect at each step of the hiring process.
