Case Manager - follow-up and outcomes (remote)

Cedar River Community Partners is looking for a Case Manager. The job centres on follow-up and outcomes for food distribution, and it would suit someone who likes steady, practical work. Mission matters here, and so does looking after staff. What you will do • Help with applications and appointments for food distribution. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Fully remote, with quarterly visits in Des Moines. • We will tell you what to expect at each step of the hiring process.
