Case Manager - follow-up and outcomes (remote)

Amberfield Keystone Care Services provides in-home and community support in Birmingham, and we need a Case Manager to lead our follow-up and outcomes. The work is personal and so is the culture. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with clear deliverables from the first week. • Fully remote, with quarterly visits in Birmingham. • We will tell you what to expect at each step of the hiring process.
