Case Manager - follow-up and outcomes (remote)

Amberfield Keystone Care Services supports families and individuals in Las Vegas with personal care and coordination, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for licensing. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Fully remote, with a monthly team day in Las Vegas. • Applications are reviewed on a rolling basis.
