Case Manager - follow-up and outcomes (remote)

We are growing our team at Cedar Bridge Care Services Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for four months, with a review at the end and a chance of extension. • Remote-friendly, with a monthly team day in El Paso. • We will tell you what to expect at each step of the hiring process.
