Case Manager - follow-up and outcomes (remote)

We are growing our team at Rivermark Vista Property Management Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would join a small organisation of about 57 people, working mostly from home. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for unit turnovers. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with unit turnovers. • Bilingual English and Spanish. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Fully remote, with an occasional day on site in Baltimore. • We reply to every applicant.
