Case Manager - follow-up and outcomes (remote)

We are growing our team at Summit Basin Civic Services and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for public records. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with public records. Working arrangement • Full-time, Monday to Friday. • Fully remote, with a monthly team day in Little Rock. • We reply to every applicant.
