Case Manager - follow-up and outcomes (remote)

Clearpath Bright Housing is looking for a Case Manager. The job centres on follow-up and outcomes for waiting lists, and it would suit someone who likes steady, practical work. Collaboration with local agencies is constant. What you will do • Help with applications and appointments for waiting lists. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Part-time, about 28 hours a week, mornings. • Remote-friendly, with an occasional day on site in Cincinnati. • We reply to every applicant.
