Case Manager - follow-up and outcomes (remote)

We are growing our team at Summit Maple Community Partners and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for food distribution. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Contract for six months, with the possibility of becoming permanent. • Remote-friendly, with quarterly visits in St. Louis. • We will tell you what to expect at each step of the hiring process.
