Case Manager - follow-up and outcomes (remote)

Summit Orchard Care Services is a social-care provider serving older adults and people with disabilities around Las Vegas, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Contract for six months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Las Vegas. • Applications are reviewed on a rolling basis.
