Case Manager - follow-up and outcomes (remote)

Fernwood Kestrel Early Learning operates neighbourhood childcare around Honolulu, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for enrolment. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Fully remote, with an occasional day on site in Honolulu. • We will tell you what to expect at each step of the hiring process.
