Case Manager - follow-up and outcomes (remote)

Greenridge Maple Property Management manages apartment communities around Portland, and we need a Case Manager to lead our follow-up and outcomes. Maintenance and leasing staff share information every day. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for vendor invoices. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with vendor invoices. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with quarterly visits in Portland. • Applications are reviewed on a rolling basis.
