Case Manager - follow-up and outcomes (remote)

Fernwood Field Learning Group runs classrooms and tutoring across the Charleston area, and we need a Case Manager to lead our follow-up and outcomes. Collaboration between teachers and staff is strong. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for student accounts. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with student accounts. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with a monthly team day in Charleston. • Applications are reviewed on a rolling basis.
