Case Manager - follow-up and outcomes (remote)

Harborlight Ridge Community Partners runs community programmes across Wichita, and we need a Case Manager to lead our follow-up and outcomes. Flexibility and kindness are expected. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for food distribution. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • Fully remote, with a monthly team day in Wichita. • Applications are reviewed on a rolling basis.
