Case Manager - follow-up and outcomes (remote)

Bluepeak Corner Housing provides affordable homes and support services around Las Vegas. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for rent assistance. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with rent assistance. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with quarterly visits in Las Vegas. • Applications are reviewed on a rolling basis.
