Case Manager - follow-up and outcomes (remote)

Summit Wilder Early Learning is looking for a Case Manager. The job centres on follow-up and outcomes for enrolment, and it would suit someone who likes steady, practical work. You would join a small organisation of about 199 people, working mostly from home. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for enrolment. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Fully remote, with a monthly team day in Jacksonville. • We will tell you what to expect at each step of the hiring process.
