Case Manager - follow-up and outcomes (remote)

Cedar Cobalt Learning Group operates schools and learning programmes around Albuquerque. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Help with applications and appointments for learning materials. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with learning materials. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • Fully remote, with a monthly team day in Albuquerque. • Applications are reviewed on a rolling basis.
