Case Manager - follow-up and outcomes (remote)

Amberfield Nest Housing manages affordable housing communities across Tampa, and we need a Case Manager to lead our follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for lease compliance. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with lease compliance. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • Remote-friendly, with a monthly team day in Tampa. • Applications are reviewed on a rolling basis.
