Case Manager - follow-up and outcomes (remote)

Fernwood Fox Community Partners is looking for a Case Manager. The job centres on follow-up and outcomes for grant reporting, and it would suit someone who likes steady, practical work. You would join a small organisation of about 82 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for grant reporting. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with grant reporting. • Bilingual English and Spanish. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with an occasional day on site in Baltimore. • We will tell you what to expect at each step of the hiring process.
