Case Manager - follow-up and outcomes (remote)

We are growing our team at Westbrook Vista Community Partners Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for volunteer shifts. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with volunteer shifts. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with a monthly team day in Virginia Beach. • We will tell you what to expect at each step of the hiring process.
