Case Manager - follow-up and outcomes (remote)

Clearpath Haven Learning Group operates schools and learning programmes around San Diego, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of more than 1,000 people, working mostly from home. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for student accounts. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with student accounts. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with an occasional day on site in San Diego. • Applications are reviewed on a rolling basis.
