Case Manager - follow-up and outcomes (remote)

Foxglove Maple Property Management manages apartment communities around Baltimore, and we need a Case Manager to lead our follow-up and outcomes. Maintenance and leasing staff share information every day. What you will do • Help with applications and appointments for lease renewals. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with lease renewals. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with quarterly visits in Baltimore. • Applications are reviewed on a rolling basis.
