Case Manager - follow-up and outcomes (remote)

Cedar Hollow Care Services provides in-home and community support in Baltimore. We are hiring a Case Manager to take on follow-up and outcomes. We train and promote from within. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for caregiver training. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for three months, with clear deliverables from the first week. • Fully remote, with an occasional day on site in Baltimore. • Applications are reviewed on a rolling basis.
