Case Manager - follow-up and outcomes (remote)

Rivermark Nest Housing manages affordable housing communities across Omaha. We are hiring a Case Manager to take on follow-up and outcomes. Collaboration with local agencies is constant. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for waiting lists. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with quarterly visits in Omaha. • We will tell you what to expect at each step of the hiring process.
