Case Manager - follow-up and outcomes (remote)

Northwind Pine Community Partners Group is looking for a Case Manager. The job centres on follow-up and outcomes for food distribution, and it would suit someone who likes steady, practical work. Mission matters here, and so does looking after staff. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for food distribution. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with food distribution. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • Fully remote, with quarterly visits in Honolulu. • We will tell you what to expect at each step of the hiring process.
