Case Manager - follow-up and outcomes (remote)

Silverline Meadow Housing is a nonprofit housing provider in Hartford. We are hiring a Case Manager to take on follow-up and outcomes. You would join a large organisation of about 954 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for waiting lists. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with waiting lists. Working arrangement • Part-time, about 28 hours a week, mornings. • Fully remote, with a monthly team day in Hartford. • Applications are reviewed on a rolling basis.
