Case Manager - follow-up and outcomes (remote)

We are growing our team at Nova Summit Early Learning and adding a Case Manager whose main responsibility will be follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for enrolment. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with enrolment. Working arrangement • Full-time, Monday to Friday. • Remote-friendly, with quarterly visits in Rochester. • We reply to every applicant.
