Case Manager - follow-up and outcomes (remote)

Lumen West Care Services provides in-home and community support in Madison. We are hiring a Case Manager to take on follow-up and outcomes. The work is personal and so is the culture. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Full-time, with a steady weekday schedule. • Fully remote, with a monthly team day in Madison. • Applications are reviewed on a rolling basis.
