Case Manager - follow-up and outcomes (remote)

Lumen Wilder Early Learning is looking for a Case Manager. The job centres on follow-up and outcomes for family communication, and it would suit someone who likes steady, practical work. You would join a small organisation of about 109 people, working mostly from home. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 16 hours a week, with flexible days. • Fully remote, with an occasional day on site in Anchorage. • We reply to every applicant.
