Case Manager - follow-up and outcomes (remote)

Bluepeak Willow Early Learning Group operates neighbourhood childcare around Minneapolis, and we need a Case Manager to lead our follow-up and outcomes. Low staff turnover is a goal we take seriously. What you will do • Help with applications and appointments for family communication. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Full-time, with a steady weekday schedule. • Remote-friendly, with an occasional day on site in Minneapolis. • We reply to every applicant.
