Case Manager - follow-up and outcomes (remote)

Redstone Juniper Care Services supports families and individuals in Oakland with personal care and coordination. We are hiring a Case Manager to take on follow-up and outcomes. You would join a small organisation of about 60 people, working mostly from home. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Part-time, about 28 hours a week, afternoons. • Remote-friendly, with an occasional day on site in Oakland. • We will tell you what to expect at each step of the hiring process.
