Case Manager - follow-up and outcomes (remote)

Summit Linden Health Group is looking for a Case Manager. The job centres on follow-up and outcomes for electronic records, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for electronic records. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Contract for four months, with the possibility of becoming permanent. • Remote-friendly, with a monthly team day in Boston. • We reply to every applicant.
