Case Manager - follow-up and outcomes (remote)

Foxglove Loom Early Learning operates neighbourhood childcare around Albuquerque, and we need a Case Manager to lead our follow-up and outcomes. You would join a small organisation of about 50 people, working mostly from home. What you will do • Help with applications and appointments for enrolment. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with enrolment. • Bilingual English and Spanish. Working arrangement • Contract for three months, with a review at the end and a chance of extension. • Remote-friendly, with an occasional day on site in Albuquerque. • Applications are reviewed on a rolling basis.
