Case Manager - follow-up and outcomes (remote)

We are growing our team at Highbridge Loom Learning Group and adding a Case Manager whose main responsibility will be follow-up and outcomes. Families are partners. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for student accounts. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with student accounts. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • Remote-friendly, with an occasional day on site in Fresno. • We reply to every applicant.
