Case Manager - follow-up and outcomes (remote)

Stonegate Rook Housing is a nonprofit housing provider in Orlando. We are hiring a Case Manager to take on follow-up and outcomes. You would work closely with colleagues in operations, support and management. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for repair requests. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with repair requests. Working arrangement • Part-time, about 24 hours a week, spread across three days. • Remote-friendly, with an occasional day on site in Orlando. • We reply to every applicant.
