Case Manager - follow-up and outcomes (remote)

Foxglove Loom Community Partners is a Cincinnati charity working with residents on basic needs and opportunity, and we need a Case Manager to lead our follow-up and outcomes. You would join a large organisation of about 984 people, working mostly from home. What you will do • Help with applications and appointments for food distribution. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with food distribution. Working arrangement • Full-time, on a regular day shift. • Remote-friendly, with a monthly team day in Cincinnati. • We will tell you what to expect at each step of the hiring process.
