Case Manager - follow-up and outcomes (remote)

Highbridge Drift Civic Services supports public agencies in Fresno with budgeting, records and programmes. We are hiring a Case Manager to take on follow-up and outcomes. Public service means patience, paperwork and pride in the outcome. What you will do • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Help with applications and appointments for budget submissions. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with budget submissions. Working arrangement • Part-time, about 20 hours a week, mornings. • Fully remote, with an occasional day on site in Fresno. • We will tell you what to expect at each step of the hiring process.
